Seattle, WA 98118   Your order cannot be processed without your shoulder patches, badge patches, rank insignias, etc! Please remember that if you are ordering the Vest with Zip Off Bolero Jacket, you need to send 2 badge patches for each jacket ordered. A badge patch goes on the Vest AND another goes on the Bolero. Please calculate accordingly when sending patches!     STEP 2  (Olympic Uniforms Order Form - Page 1) - After Completing This Form, please follow STEP 3 (Olympic Uniforms Sizing Form - Page 2).  When all three forms have been thoroughly completed, please print the forms and choose from one of these two options: 1. Fax the forms to (714) 990-2448.  Please do not do both or your order could get duplicated!       Please proceed to STEP 3 (Olympic Uniforms Sizing Form - Page 2) below...      STEP 3:  OLYMPIC UNIFORMS SIZING FORM Please take the following measurements. If you wear body armor, please take measurements with vest on  1. NAME MANDATORY  2. EMAIL MANDATORY  3. DEPT.  4. HEIGHT  5. WEIGHT 1. NECK: Measure around base of neck. 2. SHOULDERS: With arms straight down at sides, measure all the way around fullest part of shoulders. 3. CHESTw/VEST: Measure just under arms around fullest part of chest. 4. SLEEVE: Hold arm up parallel to ground, bend elbow 90 degrees, then measure from base of neck to wrist, along outside edge of arm. 5. CENTER BACK: Measure from base of neck to center of belt line. 6. WAIST: Measure around waist (PLEASE USE YOUR ACTUAL MEASUREMENT. NOT YOUR PANTS SIZE.) 7. HIPS: Measure around fullest part of buttocks or hips, whichever is larger. 8. THIGH: Measure around fullest part of upper thigh. 9. INSEAM: Measure from crotch to point on leg where pant is to stop. 10. CALF: Measure around fullest part of calf.
